 Blog 3: Scapulohumeral Rhythm      Scapulohumeral rhythm is the ratio of movement between the scapula and the humerus. This process involves two joints; the scapulothoracic (ST) joint, and the glenohumeral (GH) joint. The ST joint is a functional joint that articulates bone-muscle-bone between the scapula and thoracic wall. The GH joint is a ball and socket joint of the convex humeral head and the concave glenoid fossa. Within the scapulohumeral rhythm, the ST and GH joints form a 2:1 ratio. The shoulder has three degrees of movement, so one degree is at the ST joint while the other two are at the GH joint.       Scapulohumeral rhythm is clinically important because it allows for a large shoulder range of motion. Since scapulohumeral rhythm includes more than one joint, they work congruently, and the shoulder is able to move smoothly and in multiple planes of motion.  Biomechanics Blog 2: Test Positioning  Using bony landmarks is something that is used in measuring rang of motion (ROM) Bony landmarks give therapists a consistent and non changing reference point to start at. Every person is a different shape and size, but our bones are all in the same place. This way we are consistent when measuring. If you are measuring someones shoulder flexion every week but start at a different spot every time, the measurements will always be flawed. If you start at the same bony landmark it will give a consistent point of contact to measure from. The purpose of a proper test position for manual muscle testing (MMT) is related to the reasoning for using bone landmarks in ROM. It provides a proper start position that we can use with every single client. This is to keep the test as valid and reliable as possible.  Getting ready for the day: Reaching for a granola bar in my pantry  Each morning, I go in my pantry and reach for breakfast to take to school. I will be talking about the action of my shoulder to elevate my arm up to the shelf where my granola bars are. I start with my arm in neutral position at 0 degrees flexion by my side, but as I begin to reach up to the top shelf in my pantry, my shoulder begins to flex as my arm reaches up.  My arm starts by my side, and then begins to elevate up over my head to reach up to my food. Flexion is occurring in my shoulder joint. Flexion as well as extension occurs in the sagittal plane along the frontal axis.  My Why: I was in fieldwork for undergrad when I got my acceptance to UTHSC. My supervisor made me take a picture  with it and looking back I am SO glad he made me. Speaking to a 6 year old child:      Hi! My name is Sarah, and I will be your occupational therapist. I know this seems confusing but let me explain it. I read that you love to play on the playground, but you get nervous going up to the top of the slide. That is something we can work on here! Occupational therapy is where you will get to work on fun skills that you can bring to the playground, or doing other things you love to do. I bet your big sister can get ready for school all by herself. That is something we will work on too! We can practice buttoning your shirts and zipping up your jacket.
